Primary Job Responsibilities

  1. 1.  Ensuring Service Recipients receiving Day Services are involved in outcome related activities and that they have participated in the development of those outcomes
  2. 2.  Ensuring Service Recipients are involved in meaningful, measurable activities that relate to ISP outcomes
  3. 3.  Ensuring all staff are involved with Service Recipients throughout all activities and staff are supportive of Service Recipients in all pursuits to include community involvement and employment opportunities
  4. 4.  Serving as a primary point of contact for families of community individuals
  5. 5.  Supervise and manage all Personal Assistant (PA) homes to include schedules, assessments, and monthly home visits
  6. 6.  Complete monthly reviews for services recipients who live in the community and receive only PA, Transportation, and/or Day Services or any combination thereof
  7. 7.  Coordinate day services schedules with residential County wide Home Managers and Support Managers to ensure services are consistently provided to persons supported  
  8. 8.  Act as the primary supervisor for any staff who work day program or PA hours only
  9. 9.  Conduct Job Performance Evaluations for direct report staff
  10. 10. To monitor and review the services provided every day to ensure all staff are properly documenting program related actions and activities.  Any short comings in data collection or implementation are to be brought to the immediate attention of the Assistant County Director and/or the County Director
  11. 11. Ensure agency vehicle information, including problems and/or concerns, is promptly reported to the Assistant County Director, the County Director, and the Financial Director
